-Determining eligibility for various programs and services.
-Assisting self-advocates & families with developing and designing plans for service that are coordinated and comprehensive.
-Assisting with finding certified and experienced providers.
-Obtaining authorization for funding of services.Monitoring service quality and outcomes.
-Routinely reviewing situations for needed modifications to services.
-Assisting with initiating self-directed services.
-Assessing needs for sound financial & estate planning such as SpecialNeeds Trusts.
-Navigating Social Security and Medicaid regulations.
-Assuring health and safety.
No Boundaries provides case management services in several counties throughout the state of Kentucky that includes but is not limited to Lexington, Somerset, Frankfort, Elizabethtown, Corbin, Nicholasville, Winchester, Morehead, Covington, Springfield, Lawrenceburg, Georgetown, Versailles, Louisville, and Richmond.
No Boundaries case management currently offers services through the Supports for Community Living Waiver (for more information about this waiver, click our Resources tab!) in Kentucky. We serve families who utilize traditional SCL waiver supports, Participant Directed Supports (PDS), as well as Michelle P. Waiver supports. Our team has decades of combined experience in assisting families with navigating through the often times confusing process of establishing waiver services. We pride ourselves on a person centered team approach that centers around the individual and their vision for their future.
